- 0:01Lines every morning luckily skin aging
- 0:04is something that can easily and rapidly
- 0:06be reversed with new tools available to
- 0:08us today in this video I'll show you a
- 0:10step-by-step system backed by the latest
- 0:12scientific evidence to reverse the signs
- 0:14of skin aging how fast you'll see
- 0:16results and other skin treatments that
- 0:18are on the horizon and this is not just
- 0:20for vanity reasons how our skin looks
- 0:23reflects our chances of living a long
- 0:25life here's a step-by-step guide to
- 0:27rejuvenate your skin quickly first we
- 0:30need to stop the damage there's no point
- 0:32in trying to heal the skin if it's only
- 0:34going to become wrinkled and damaged
- 0:36again so there are two types of Aging
- 0:38processes intrinsic and extrinsic aging
- 0:41and we need to address both starting
- 0:43with intrinsic aging intrinsic or
- 0:46chronological aging affects the skin of
- 0:48the entire body it makes the skin appear
- 0:51dry pale thin and transparent epidermal
- 0:54turnover is also slowed down and it
- 0:56takes longer for new skin cells formed
- 0:58at the deeper layers of the skin to
- 1:00migrate to the surface we have decreased
- 1:02blood supply and these changes
- 1:04contribute to delayed wound healing the
- 1:06amount of dermal collagen may be
- 1:08decreased by up to 75% with age and the
- 1:11remaining collagen is fragmented and
- 1:13disarrayed senescent cells accumulate in
- 1:15the skin of older people and there's a
- 1:17decrease in subdermal fat this loss of
- 1:20support contributes to skin wrinkling
- 1:22and sagging as well as increase
- 1:24susceptibility to trauma so what do we
- 1:26do about intrinsic aging how do we slow
- 1:29down in ideally stop these changes in
- 1:30the skin well there are two creams and
- 1:33three supplements with strong evidence
- 1:35the first step is retinoid creams so the
- 1:38term retinoid refers to synthetic and
- 1:40natural analoges of vitamin A these
- 1:43creams can stimulate the activity of our
- 1:45skin cells particularly cartinos sites
- 1:47and vibr blasts so think of retinoid
- 1:50creams as personal trainers for your
- 1:52skin cells so just like a trainer helps
- 1:54strengthen and tone your muscles
- 1:55retinoids push your skin cells to work
- 1:58harder promoting skin turnover over and
- 2:00boosting collagen production retinoid
- 2:02creams also strengthen the Skin's
- 2:04protective function reduces water loss
- 2:06and inhibits metella protases so matell
- 2:09protases are a group of enzymes that
- 2:11break down proteins and they break down
- 2:14the extracellular Matrix not good
- 2:16retinoid creams help protect against
- 2:18this finally retinoid creams enhance
- 2:21remodeling of reticular fibers and
- 2:23stimulate the growth of new blood
- 2:25vessels in the deeper layers of the skin
- 2:27and multiple randomized control trials
- 2:30have confirmed these effects such as
- 2:32this 2007 study retinol lotion or a
- 2:35placebo was applied to either the right
- 2:37or the left arm and after 24 weeks there
- 2:39were significant differences between the
- 2:41retinol treated and Placebo treated skin
- 2:44in their wrinkling scores these
- 2:45improvements were accompanied by
- 2:47increases in Matrix molecules
- 2:49procollagen and the strengthening of
- 2:51extracellular Matrix but when first
- 2:53using retinoid creams it does make the
- 2:55skin more sensitive and it can inflame
- 2:57the skin so these creams are normally
- 2:59used used at night and the
- 3:01recommendation is to always wear
- 3:02sunscreen during the day and we'll come
- 3:04back to sunscreen later in the video
- 3:06pregnant women should also not use these
- 3:08creams and broadly speaking there are
- 3:10two common types of retinoid creams
- 3:12we've got adapalene and trinoline
- 3:15adapalene is the least irritating
- 3:16topical retinoid and it's more targeted
- 3:19to The receptors in the skin whereas
- 3:21tanoan is an earlier version and it
- 3:23generally causes more irritation to the
- 3:25skin but which type is better for skin
- 3:28aging well in a head-to-head trial in
- 3:302018 both adapalene and tanoan were
- 3:33equally effective at reversing the signs
- 3:35of skin aging so adapalene is probably
- 3:38preferred since it's less irritating to
- 3:40the skin and it works just as well but
- 3:42in New Zealand we don't have access to
- 3:44the higher strength 0.3% adapalene so I
- 3:47use tanoan 0.05% at night but not every
- 3:51night and this brings me onto the next
- 3:52cream that I use on to step two lactic
- 3:56acid and glycolic acid are exfoliants
- 3:58that when used over the longterm affect
- 4:00the deeper layers of the skin and reduce
- 4:02wrinkles the clinical guidelines note
- 4:04that these acids help collagen and
- 4:06elastin to regenerate making Fine Lines
- 4:09less obvious and these recommendations
- 4:11are based on Trials such as this one
- 4:13where treatment with lactic acid
- 4:15resulted in improvements in skin
- 4:16firmness thickness and improved
- 4:18smoothness and appearance of lines and
- 4:20wrinkles lactic acid and glycolic acid
- 4:23are known as Alpha hydroxy acids and
- 4:25there's also beta hydroxy acids such as
- 4:27salicylic acid both Alpha and beta
- 4:30hydroxy acids help in the removal of
- 4:32dead skin cells and promote skin renewal
- 4:34so on Monday and Thursday nights I apply
- 4:37both the alpha and beta hydroxy acids
- 4:39and I leave them on overnight but on
- 4:41those nights I don't apply the retinoid
- 4:43cream to finish off the intrinsic aging
- 4:46section before moving on to extrinsic
- 4:48aging the third step is supplements
- 4:50specifically three supplements with
- 4:52strong evidence of benefit and I've
- 4:54recently made a video about all three of
- 4:56them here but in short they are collagen
- 4:58peptides 10 to 15 G hyaluronic acid 200
- 5:02mg and vitamin B3 but not in mega doses
- 5:05the hyaluronic acid and vitamin B3 I
- 5:08take as part of
- 5:10micrites supplement or use any of these
- 5:12skin creams does not in any way mean
- 5:14that you should as well now let's have a
- 5:17look at extrinsic aging of the skin
- 5:19extrinsic aging is related to
- 5:21environmental factors and it was first
- 5:23reported at the end of the 19th century
- 5:25and it was then described as farmer skin
- 5:27or sailor skin extrinsic generally
- 5:30affects the exposed areas of the body
- 5:32such as the face the neck and the arms
- 5:34and the features of photo damage include
- 5:37roughness deep wrinkling dis
- 5:39pigmentation and Tangi Tasia which are
- 5:41small visible red blood vessels and they
- 5:43represent broken capillaries so how do
- 5:46we treat extrinsic aging of the skin
- 5:48well the fourth step in reversing the
- 5:50signs of skin aging is diet and as the
- 5:53clinical guidelines point out nutrition
- 5:55is known to affect extrinsic aging now
- 5:57no diet is going to be perfect for every
- 5:59one but there are some Diet fundamentals
- 6:02that are associated with improved skin
- 6:04Health a Mediterranean style diet rich
- 6:06in plant-based Foods featuring plentiful
- 6:09fresh fruit and vegetables herbs nuts
- 6:11beans and whole grains with moderate
- 6:13amounts of seafood Dairy poultry and
- 6:16eggs and occasional red meat is
- 6:18associated with good skin Health but on
- 6:20the other hand higher fat and
- 6:22carbohydrate intake has been associated
- 6:24with an increased risk of wrinkles and
- 6:26skin atrophy again no diet is perfect
- 6:29for every one and I encourage my
- 6:30patients to have a diet that's high in
- 6:32lean protein plenty of fiber from Whole
- 6:35fruits and non-starchy vegetables
- 6:37unsaturated fats like extra virgin olive
- 6:39oil walnuts almonds avocados and some
- 6:42salmon the fifth step is exercise and a
- 6:45really interesting paper came out in
- 6:47June last year that looked at resistance
- 6:49training and its effects on skin aging
- 6:52it was a 16-week randomized control
- 6:54trial and it found that resistance
- 6:56exercise counteract skin aging such as
- 6:59deterior ation in skin elasticity upper
- 7:01dermal structure and dermal thickness
- 7:03aerobic training also had positive
- 7:05effects on skin elasticity but it did
- 7:07not improve dermal thickness and it
- 7:09emphasizes the point that in addition to
- 7:11aerobic exercise we need to do
- 7:13resistance exercise we have to lift
- 7:16weights and emphasize that our diet on
- 7:18the inside is projected to our skin on
- 7:20the outside the next step is sleep a
- 7:232021 metaanalysis found what we already
- 7:26intuitively know a lack of sleep
- 7:28accelerates the signs of skin aging so
- 7:30think of sleep as your nightly repair
- 7:32session for your skin during deep sleep
- 7:34your body goes into overdrive repairing
- 7:37damage and rejuvenating your skin
- 7:39skimping on sleep is like skipping
- 7:40maintenance on a machine it starts to
- 7:42show wear and tear quickly the same was

- 7:47Management and the final two steps to
- 7:49treat extrinsic aging before we have a
- 7:51look at more advanced strategies to
- 7:53reverse the signs of skin aging are some
- 7:55further skin creams so the eighth step
- 7:58overall is a moisturized with ceramides
- 8:00ceramides are essentially the glue that
- 8:02holds our skin cells together and helps
- 8:04keep our Skin Barrier intact and healthy
- 8:07in 2019 a randomized control trial
- 8:10showed that ceramides significantly
- 8:12decreased wrinkles and improved skin
- 8:14texture the second cream and it's the
- 8:16most important cream of all that I've
- 8:18mentioned so far is sunscreen but it's
- 8:20crucial to pick a sunscreen with a
- 8:22certain ingredient in it but before I
- 8:24get on to that here's a striking photo
- 8:26of a 92-year-old who used sunscreen on
- 8:28her face but not her neck think of your
- 8:31skin as a delicate piece of fabric just
- 8:33as constant exposure to sunlight can
- 8:35fade and damage that fabric UV rays can
- 8:38cause significant harm to the skin using
- 8:40sunscreen is like putting a protective
- 8:42cover over that fabric and it prevents
- 8:44wear and tear the only push back I've
- 8:46seen against sunscreen that I've seen
- 8:48online is safety which we'll cover
- 8:50shortly but first in a landmark 2013
- 8:53trial of 93 adults the group who took
- 8:56sunscreen daily had no detectable
- 8:58increase in their skin AG aging after 4
- 9:00and A2 years and a follow-up 2016 study
- 9:03showed the reversal of signs of skin
- 9:05Aging in the group who used sunscreen
- 9:07but there are concerns with some
- 9:09sunscreen ingredients so broadly we have
- 9:11mineral sunscreens and chemical
- 9:13sunscreens while chemical sunscreen
- 9:15ingredients offer better protection
- 9:17against UV radiation they're easier to
- 9:19apply and are waterresistant most
- 9:21chemical sunscreen ingredients can be
- 9:23absorbed through the skin and that's
- 9:25created a worry that those ingredients
- 9:27can disrupt hormone levels those
- 9:29concerns are based on single cell and
- 9:31mice studies where extremely high doses
- 9:33of those ingredients are given and yes
- 9:35there are changes to hormone levels and
- 9:38responses however studies in humans have
- 9:40been reassuring with no evidence for
- 9:42endocrine effects in humans that's
- 9:45because the amount of these ingredients
- 9:46that enter the bloodstream in humans is
- 9:48minute and the American Academy of
- 9:50Dermatology concluded that the science
- 9:52doesn't show that any of the sunscreen
- 9:54ingredients currently available in the
- 9:56US are harmful to human health
- 9:59nevertheless Some people prefer to use
- 10:01mineral sunscreens such as zinc oxide as
- 10:04those ingredients while they don't offer
- 10:05as good UV protection they don't get
- 10:07absorbed and alter MD is a popular
- 10:10choice however there is a chemical
- 10:12ingredient that offers The Best of Both
- 10:14Worlds called bitol it offers fantastic
- 10:17UV protection but it's a large molecule
- 10:19and it doesn't get absorbed through the
- 10:21skin there's no other sunscreen
- 10:23ingredient on the planet that's been
- 10:24tested as much as this one and it's
- 10:27hoped that bitol will be FDA approved d
- 10:29by the end of 2025 now it's been widely
- 10:32used overseas in Europe for decades but
- 10:34it's just the FDA approval that we're
- 10:36waiting on for use in the USA so if
- 10:39you're living in the US there are ways
- 10:40to get batal sunscreens if you go
- 10:42looking and that's all I can say on the
- 10:44matter I apply a moisturizer first in
- 10:47the morning and then wait a couple of
- 10:48minutes then I apply the sunscreen so
- 10:51this nine-step system to reverse the
- 10:53signs of skin aging uses the latest
- 10:55scientific evidence and patients
- 10:57generally notice a significant
- 10:58Improvement in the skin after following
- 11:00the system for about 30 days but for
- 11:03those who want to know additional
- 11:04strategies here are some more advanced
- 11:06options that have great evidence in per
- 11:09menopausal women hormone replacement
- 11:11therapy May delay skin thinning the skin
- 11:13is less dry with fewer wrinkles and the
- 11:15wound healing is faster than prior to
- 11:17treatment Botox injections are another
- 11:20option as are laser treatments
- 11:22photodynamic therapy radio frequency
- 11:24devices and possibly red light therapy
