  2. 0:01I'm a pharmacist in this video I want to
  3. 0:03talk about testosterone gel as usual
  4. 0:06this is a viewer question that was
  5. 0:09submitted to me so I kind of want to
  6. 0:10give you a 35,000 foot overview we'll
  7. 0:12talk about how effective it is uh does
  8. 0:16it benefit things how it's best Ed side
  9. 0:19effects risk factors all those sorts of
  10. 0:21things so let's just hop right into it
  11. 0:23now first of all testosterone gel goes
  12. 0:25by a number of different brand names
  13. 0:27probably familiar with andrel it is
  14. 0:29available generic worked out but let's
  15. 0:31talk about Effectiveness okay so the
  16. 0:34main study that was done they took 274
  17. 0:37men these men ranged in age from 18 to
  18. 0:4080 but they all had low testosterone
  19. 0:42levels there was a wide range in their
  20. 0:45body mass index so as you're probably
  21. 0:48aware how heavy somebody is how much fat
  22. 0:51they're carrying has a big effect on
  23. 0:53their testosterone levels so it wasn't
  24. 0:55the best design study in the world but
  25. 0:57it gave us a good idea of what the gel
  26. 0:59could do for people they started them
  27. 1:01out at two pumps which is 40.5 mg of the
  28. 1:06testosterone gel per day and then they
  29. 1:09they adjusted that dose and looked at it
  30. 1:11older
  31. 1:131428 in 112 days can what they found is
  32. 1:17that the men on the placebo came in at
  33. 1:19an average testosterone level of 303 one
  34. 1:23p for 20.25 mg was
  35. 1:27457 the two pumps which is the normal do
  36. 1:30will talk about that in a second was
  37. 1:33524 n per dcil and then at
  38. 1:376.75 was actually the highest level at
  39. 1:41643 n per deciliter interestingly the
  40. 1:45highest dose did not have the highest
  41. 1:47testosterone level 81 mg was
  42. 1:51537 so it looks like three pumps was
  43. 1:54kind of the ideal level at 112 days
  44. 1:58interestingly these men could choose to
  45. 2:00continue for a year and what they found
  46. 2:03is that their average level slowly
  47. 2:05declined the rest of the year so if
  48. 2:07you're going to do that you definitely
  49. 2:09want to keep up with your blood
  50. 2:11measurement because you may need to
  51. 2:12increase that dose as it goes along now
  52. 2:15what about side effect effect fethers
  53. 2:16defitely side effects with testosterone
  54. 2:18gel the most common the one you need to
  55. 2:20be aware of is an increase in prostate
  56. 2:23specific antigen we can see this in over
  57. 2:2511% of men who take it so be wary of
  58. 2:29that also there were emotional issues um
  59. 2:32you know the old like hey if you're
  60. 2:34going to take testosterone you can get
  61. 2:37angry Outburst those sorts of things it
  62. 2:39does happen not for the reason people
  63. 2:42think and I can do that on another video
  64. 2:44but so the emotional component 2.6% of
  65. 2:48men and then high blood pressure
  66. 2:50occurred and just over 2% so those are
  67. 2:53the main things to watch out for as far
  68. 2:54as risk factors we kind of touched on it
  69. 2:58any sort of prostate he answer obviously
  70. 3:01dph is benine prostatic hypertrophy
  71. 3:04that's a concern something you want to
  72. 3:06keep an eye out for secondary exposure
  73. 3:08what does that mean it means if you
  74. 3:10apply this gel on a area of your skin
  75. 3:13that can touch somebody else whether
  76. 3:14it's your wife or kids what have you
  77. 3:17they can pick that up so we'll talk
  78. 3:19about how and where to apply it but
  79. 3:21that's a risk factor blood plotting
  80. 3:24cardiovascular risk ininal what they
  81. 3:27call the tits you can get that
  82. 3:29when you apply testosterone so that is a
  83. 3:32risk factor as well hey now let's talk
  84. 3:34about dosing how do you dose it what's
  85. 3:36the best way to do it there's a lot of
  86. 3:38information out there some of it's not
  87. 3:40so good so what we usually do we're
  88. 3:42going to start people normally at two
  89. 3:44pumps per day or 40.5 Mig a day and then
  90. 3:49that half M 14 day and then 28 day Mark
  91. 3:53that's where we're going to adjust the
  92. 3:54dose everybody's going to be different
  93. 3:56so you do want to keep up with that keep
  94. 3:58in mind all right
  95. 4:00you want to when you dose because you're
  96. 4:03going to dose every day you want to keep
  97. 4:05that away from when you shower because
  98. 4:08if you shower you know a couple of hours
  99. 4:11after you apply your dose your blood
  100. 4:13levels are going to be lower so try to
  101. 4:15space out when you apply and when you
  102. 4:18shower by probably 10 hours 10 12 hours
  103. 4:21so if you apply in the morning try to
  104. 4:22shower in the evening it sunscreen
  105. 4:25interestingly also moisturizing lotion
  106. 4:28has been shown to increase your levels
  107. 4:31so if you're somebody who applies that
  108. 4:33lotion and your levels come back high
  109. 4:35that might be wise so just let your talk
  110. 4:38okay best results now we talked about
  111. 4:41the dose how do you apply it to get the
  112. 4:44best results okay so first rule clean
  113. 4:48dry intact skin intact means if you have
  114. 4:51a rash somewhere don't apply it there
  115. 4:53okay but clean dry intact primarily
  116. 4:56we're looking at the upper arm shoulder
  117. 4:58area okay surface area this is
  118. 5:01important you don't you're going to
  119. 5:03apply the gel with the palm of your hand
  120. 5:05that's what's recommended you don't want
  121. 5:07to do a small area you actually want to
  122. 5:10apply it probably in that type of area
  123. 5:13Okay so make sure you're covering enough
  124. 5:15surface area the other thing you'll see
  125. 5:18some people recommend to apply it to
  126. 5:20different places in your body don't do
  127. 5:22that that's not what was done in studies
  128. 5:25not see it couldn't work in other areas
  129. 5:28but this is where we know we get the
  130. 5:29best results so that means no do not
  131. 5:32apply to your belly do not apply to your
  132. 5:34genitals chest armpits knees those sorts
  133. 5:37of areas focus on the upper arm
  134. 5:39shoulders okay now how are you going to
  135. 5:41get the best results number one we talk
  136. 5:43about this your dosings important and
  137. 5:46you need to keep up with your blood
  138. 5:47draws otherwise you don't know if the
  139. 5:49dose needs to go up or down so that 2 we
  140. 5:52to one month hyre period is important
  141. 5:55you're probably going to see the best
  142. 5:57results as far as your maximum blood
  143. 5:59levels around that or month Mark again I
  144. 6:02mentioned earlier your maintenance do
  145. 6:05after that point we did see the level
  146. 6:08slowly decrease if you stay on the same
  147. 6:10dose so you're going to need to do your
  148. 6:12follow-up Labs you may need to see your
  149. 6:14dose go up okay so let me know in the
