31. 1:14he had taken drugs he had overdosed
32. 1:15there was nothing suspicious there

37. 1:44the longshore of it he was able to he
38. 1:46was allowed to because they could have
39. 1:48caught him way before if they'd looked
40. 1:50into anything at all then they could
41. 1:52have saved other lives there were a
42. 1:54range of mistakes i think there was a
43. 1:56combination of a need for more
44. 1:58professional curiosity so really
45. 2:01thinking through are there alternative
46. 2:03explanations for what has happened and
47. 2:05then there were some inquiries that
48. 2:07should have been carried out more
49. 2:09quickly and perhaps more thoroughly
50. 2:11those came together in a dreadful way
51. 2:14really to let down the families of these
52. 2:17victims and these victims themselves and
53. 2:19i'm tremendously sorry about that jack
54. 2:22should still be here
55. 2:24how many missed opportunities they had
56. 2:25to catch him
