Recovery Groups in Tox Studies: 4 Mistakes That Cost You Reversibility — Transcript

- 0:19finding that doesn't reverse, which is most definitely going to be called adverse.
- 0:47So the first mistake that sponsors make when they add recovery groups,
- 0:50so the first mistake that sponsors make when they add recovery groups to their
- 0:54study designs is that they only add them to the control group and the
- 0:58high ghost group. And this is usually a recommendation from the CRO to reduce
- 1:02animals and optimize a study, but I've seen it several times before where you
- 1:07will have findings in the in-life phase, and you can't tell then whether those
- 1:11findings become reversible. The FDA is definitely going to flag this,
- 1:15particularly if your NOAEL is the mid or the high dose.
- 1:18And it's also going to play into the calls and establishment of adversity,
- 1:21because if you have a finding that is not reversible,
- 1:24that is more likely to be adverse than a finding that is reversible.
- 1:27But if you don't have the animals to show reversibility,
- 1:30then you're not going to know if you're able to establish that sponsors make
- 1:32is that the recovery duration is too short.
- 1:36So a lot of times pathology and any other end points that you're using
- 1:38within these study designs, they have to align between those different animals.
- 2:00Another mistake the sponsors will make when they add recovery groups is that they'll
- 2:03treat the recovery animals differently from your main animals.
- 2:06And so they'll use different sex ratios or different terminal end points.
- 2:09And this is very common with clinical pathology.
- 2:12sometimes because you have 10 animals in your main group and
- 2:16this is very common with clinical pathology animals you might have different time points
- 2:21in your recovery animals but this is a big no-no.
- 2:24Remember that your recovery that's still a total of 15 animals per sex total
- 2:27that's going to be going through the dosing period. So you want to make
- 2:31sure that your endpoints, anything that you're adding onto the study and your study
- 2:34design, they align between the different animal groups.
- 2:39And the final mistake that a lot of duration of recovery will make is
- 2:42that they don't include TK in the recovery period.
- 2:44We talked before about how large they have longer half-lifes.
- 2:49And even for small molecules, they might have longer half-lifes,
- 2:52too, where you have to show that that drug actually has decreased in concentration,
- 2:55plasma concentration, over will align for all animals,
- 2:58and four, your TK will be included throughout Maine,
- 3:02and four, your TK will be included throughout your
- 3:10groups as well. For two, your recovery duration will meet or
- 3:14exceed the several half-life requirement. recovery period will cover several half-lifes.
- 3:20Three, you will have endpoints aligned for toxicology study design looks like where the
- 3:24recovery groups are designed And so this is what a good toxicology
- 3:29design looks like where recovery groups are put in there for a purpose and
- 3:33where the design looks good. And you're going to be able to make a
