- 0:37information, because there is a lot of
- 0:39content here. So come along with me.
- 0:47Rheumatic diseases or rheumatism. What
- 0:50is that? I constantly receive questions
- 0:53in my office, on Instagram, on WhatsApp
- 0:55, and here on YouTube, with people
- 0:57asking: "Doctor, I was diagnosed with
- 0:59rheumatism, or my mother or my son has
- 1:01rheumatism." Well, I have to understand
- 1:05how my immune system works to
- 1:07understand a little bit about what
- 1:09these rheumatic diseases are. Folks, we
- 1:12have three types of cells in our blood.
- 1:15We have red blood cells that transport
- 1:17oxygen. We have platelets that are
- 1:21responsible for clotting, but also
- 1:23responsible for all the healing factors
- 1:25in our body. And we have leukocytes or
- 1:29white blood cells. What do leukocytes
- 1:32do? They are our defense, like our
- 1:35police force. They are responsible for
- 1:39taking care of our body. So, every time
- 1:42we are attacked by a virus, a bacterium
- 1:46, or a foreign body. What do you mean
- 1:48by a foreign body? So, when a thorn
- 1:50gets into your finger, how does that
- 1:52work? Our defense system, through its
- 1:56soldiers, signals to the central hub.
- 2:00Look, we need to activate and send
- 2:02leukocytes there to be able to fight
- 2:04this invading agent. So, every time our
- 2:09body is attacked, leukocytes spring
- 2:11into action and produce antibodies.
- 2:14What are antibodies? They are like
- 2:17shots fired against the attackers. And
- 2:20what happens then with rheumatism,
- 2:22folks? Our organism—we don't know if
- 2:26this is a genetic cause, if it's a
- 2:28viral infection that might alter it, or
- 2:31if it could be an allergic mechanism.
- 2:35What happens is that our leukocytes
- 2:39start firing antibodies against our own
- 2:42organs. So, we have several types of
- 2:46rheumatism. So it begins to attack, for
- 2:49example, rheumatoid arthritis. It is a
- 2:53type of rheumatism that produces an
- 2:55antibody which often attacks the joints
- 2:57. So it begins to cause deformity in
- 3:01the hands, feet, and knees, and
- 3:04destroys the joints. Because these
- 3:08antibodies, folks, when they are inside
- 3:10the joints, they trigger inflammatory
- 3:13symptoms. So they lead to inflammation
- 3:18of that joint, leading to redness, pain
- 3:21, swelling, and joint destruction. So
- 3:26the big problem with rheumatism, with
- 3:29certain types of rheumatism, is that
- 3:32when I start having crises, crises,
- 3:34crises, these antibodies inside the
- 3:37joints lead to joint wear. Leading to
- 3:41what? To practically the loss of this
- 3:44joint and leading to arthrosis, which
- 3:46is this premature wear. So, rheumatoid
- 3:51arthritis, as I mentioned, specifically
- 3:53attacks the organs of the articular
- 3:55system, but it also attacks the lungs,
- 3:58the heart, the eyes, it attacks the
- 4:00body as a whole. But I have other types
- 4:03. For example, I have psoriasis, which
- 4:06is a type of rheumatism. It often
- 4:09attacks the skin, the scalp, and the
- 4:11joints of the elbows and knees.
- 4:14Normally, psoriasis appears a lot in
- 4:16folds of the skin. Then we have another
- 4:18type of rheumatism called lupus. What
- 4:21does lupus do? It attacks organs as a
- 4:24whole, but lupus has a characteristic
- 4:26of attacking the face. So, sometimes we
- 4:29have what we call a butterfly wing.
- 4:31Butterfly wing lesion. We have several
- 4:34other types of rheumatism. For example,
- 4:36I have a rheumatism called Sjögren's
- 4:37syndrome. In Sjögren's syndrome, do
- 4:39you know what they attack? Our glands,
- 4:42especially the lacrimal glands. So the
- 4:45eye starts to get dry, and the salivary
- 4:48glands, the mouth starts to get dry. So
- 4:52our body begins to attack antibodies
- 4:55against our organs through leukocytes.
- 5:00And what is the major causal factor for
- 5:03this? The big problem is that our
- 5:06emotional state is extremely affected
- 5:09when we start to have the manifestation
- 5:11of rheumatic diseases. How so, Tangui?
- 5:17The person, most of the time, when they
- 5:20have some intense stress, so it could
- 5:22be emotional stress from some factor,
- 5:25if they get too much sun, too much cold
- 5:28, if they sometimes overeat, every time
- 5:31they have an excess, most of the time
- 5:34these leukocytes are activated to fight
- 5:37, let's say, the external factor, but
- 5:40they start fighting our own body,
- 5:44leading to pain from limitation and
- 5:46inflammation. So, folks, when a person
- 5:49has symptoms of rheumatism, why do we
- 5:52need to seek medical help? Most of the
- 5:56time, we have a specific doctor who
- 5:57works with this rheumatological area,
- 5:59which is the rheumatologist. So, they
- 6:02work by identifying the type of
- 6:04rheumatism that this person may have.
- 6:08Nowadays, we have drugs or medications
- 6:11called immunosuppressants. And now we
- 6:15also have what is called an
- 6:16immunomodulator. What can it do? The
- 6:19immunosuppressant, most of the time, is
- 6:21a corticosteroid or a non-hormonal
- 6:23anti-inflammatory. It inhibits the
- 6:27action of antibodies on the organs.
- 6:30These are the immunosuppressants. Now,
- 6:33the immunomodulators, which are more
- 6:35modern drugs, what do they do? They are
- 6:38able to modulate the leukocytes, making
- 6:40them understand that this is a friend.
- 6:44So, we are able to modulate the
- 6:46inflammatory response of the leukocytes
- 6:48. So, when a person starts having any
- 6:51symptom of rheumatism, the first thing
- 6:53is to seek medical help. But then I go
- 6:56back again to the links of health. When
- 6:59the person is already under follow-up,
- 7:02I need to connect all the links of
- 7:04health. Why? If I do physical
- 7:08activities regularly, I decrease the
- 7:11incidence of rheumatic crises. If I
- 7:15sleep properly, this is very important,
- 7:18if the person doesn't sleep properly, I
- 7:20can intensify these crises. Adequate
- 7:24nutrition, obesity is closely related
- 7:27to inflammation. If the person can
- 7:30control their weight, control their
- 7:33diet, I can control the rheumatic
- 7:36crises. And also, folks, what I told
- 7:39you, the emotional part, the rheumatism
- 7:41part, it is closely related to
- 7:43emotional control. So, whenever I can
- 7:49invest in techniques like mindfulness,
- 7:51meditation, attending a religion so
- 7:54that it gives me emotional support,
- 7:56having people around, all of this helps
- 7:59in controlling rheumatism. So, folks, I
- 8:05hope you understood a little bit about
- 8:07the path we need to follow to identify
- 8:09who has this rheumatic disease, control
- 8:12it, and live a little better. If you

- 8:23information, many tips on quality of
- 8:25life, on well-being, so that you, your
- 8:27family, your friends can live better
