  110. 6:45hello Mrs Sutton come in how are you
  111. 6:48settling in next door have all your
  112. 6:50things from Canada arrived yet I thought
  113. 6:53I saw a removals fan outside your house
  114. 6:55yesterday afternoon yes they came
  115. 6:58yesterday we spent all day yesterday
  116. 7:01arranging them it's beginning to feel a
  117. 7:04bit more like home now oh that's good
  118. 7:06look come in and sit down are you all
  119. 7:09right
  120. 7:10you look a bit worried well I am a bit
  121. 7:13I'm sorry to bother you so early Mrs
  122. 7:16Smith but I wonder if you could help me
  123. 7:18could you tell me how I can get hold of
  124. 7:21a doctor our daughter Anna isn't very
  125. 7:24well this morning and I may have to call
  126. 7:27somebody out she keeps being sick and I
  127. 7:31am beginning to get a bit worried I just
  128. 7:33don't know how the health system works
  129. 7:35here in England all I know is that it's
  130. 7:38very different from ours back in Canada
  131. 7:40well I don't know really where to start
  132. 7:44let me think
  133. 7:46well the first thing you have to do is
  134. 7:49find a family doctor
  135. 7:51sometimes we call them General
  136. 7:53Practitioners as well right and register
  137. 7:56with him or her if you live here you've
  138. 8:00got to be on a doctor's list
  139. 8:02if you're not things can be a bit
  140. 8:04difficult nobody will come out to you if
  141. 8:07you're not registered anyway the work in
  142. 8:10things called practices sort of small
  143. 8:13groups of family doctors all working
  144. 8:15together in the same buildings now what
  145. 8:18you've got to do this morning is
  146. 8:20register with one of them there are two
  147. 8:23practices near here so we're quite well
  148. 8:25off the doctors in this part of
  149. 8:27Manchester there's the dean End Health
  150. 8:29Center about 10 minutes walk away and
  151. 8:34there's another practice in South Hay
  152. 8:36that's about five minutes away going
  153. 8:39towards the Town Center we're registered
  154. 8:42at the dean end one but they're both
  155. 8:46okay
  156. 8:47there are about six doctors in our
  157. 8:49practice and four in the other so ours
  158. 8:52is quite big in comparison and the
  159. 8:55building and everything's a bit more
  160. 8:57modern South a is a bit old-fashioned
  161. 9:00but the doctors are okay their only
  162. 9:03problem is that they don't have a proper
  163. 9:05appointment system sometimes you have to
  164. 9:08wait for ages there to see someone
  165. 9:10anyway you go to the receptionist in
  166. 9:13whichever Health Center and ask her to
  167. 9:16register you with a doctor there you
  168. 9:18have to fill in a form but it doesn't
  169. 9:20take long ours is called Dr Jones and
  170. 9:24we've been going to him for years ever
  171. 9:27since we moved here 15 years ago I
  172. 9:31wouldn't say he's brilliant but um I
  173. 9:34suppose he's all right really
  174. 9:36where you swim now they say he's very
  175. 9:38good with elderly people but he does
  176. 9:40tend to get a bit impatient with
  177. 9:42children
  178. 9:43listen the one who's supposed to be
  179. 9:46really good with small children is Dr
  180. 9:48Shaw I've Heard lots of people say that
  181. 9:51she's young and she's got small children
  182. 9:53of her own so you could try registering
  183. 9:56with her
  184. 9:58and if her lists fall I heard somebody
  185. 10:00say the other day that there's a really
  186. 10:02nice young doctor at South Hay a Dr
  187. 10:05Williams
  188. 10:07he holds special clinics for people with
  189. 10:09back trouble but that's not really your
  190. 10:12problem is it
  191. 10:16now you have some time to look at
  192. 10:18questions 18 to 20.
  193. 10:36now listen to the rest of the

  196. 10:44if you want a doctor to visit you at
  197. 10:46home you have to ask for a home visit
  198. 10:48you're supposed to do that before 10 30
  199. 10:51in the morning but obviously if it's an
  200. 10:54emergency you can phone at any time
  201. 10:56night or day it might not be your doctor
  202. 11:00that comes though it's quite often one
  203. 11:02of the other doctors in the practice it
  204. 11:04doesn't really seem to make much
  205. 11:05difference otherwise you make an
  206. 11:08appointment to see your doctor at the
  207. 11:09health center you usually get seen the
  208. 11:12same day not always of course but
  209. 11:15usually as I say the whole surgery is
  210. 11:18between 9 and 11 30 every weekday and
  211. 11:21from 4 to 6 30 Monday to Thursday
  212. 11:25Saturdays are only for emergencies when
  213. 11:28the doctor sees you it gives you a
  214. 11:30prescription it writes what medication
  215. 11:32you need on it and you take it to a
  216. 11:34chemist shop
  217. 11:35there's one opposite the center if it's
  218. 11:39for a child under 16 you don't have to
  219. 11:41pay so if it's for Anna there's no
  220. 11:43problem
  221. 11:44the same thing goes if you're unemployed
  222. 11:47or retired or if you're pregnant just as
  223. 11:50well because it's not cheap you pay the
  224. 11:53same price for each item the doctor has
  225. 11:55prescribed at the moment it's something
  226. 11:57like five pound per item so you pay for
  227. 12:00the medication but the consultation with
  228. 12:02the doctor doesn't cost you anything
  229. 12:04it's completely free as long as you're a
  230. 12:07resident here we are going to be here
  231. 12:09for three years aren't you uh-huh so
  232. 12:12well it shouldn't be any question of you
  233. 12:14paying anything to see the doctor so
  234. 12:16that's one less problem to worry about
  235. 12:19look Mrs Sutton if you want I'll sit
  236. 12:22with your daughter for half an hour if
  237. 12:24you want to go down to the Health Center
  238. 12:25to register it's no trouble really don't
  239. 12:28worry are you sure you wouldn't mind
  240. 12:30that would really help me a lot I'll ask
  241. 12:33them if they can send someone around
  242. 12:34later to see Anna I I think I'll try the
  243. 12:37dean end Center good idea don't worry

  264. 14:36well Jonathan as we explained in your
  265. 14:38letter in this part of the interview we
  266. 14:41like to talk through your application
  267. 14:42form your experience to date Etc and
  268. 14:46then in the second part you go for a
  269. 14:48group interview group interview yes I
  270. 14:51understand
  271. 14:52so your first degree was in economics
  272. 14:55yes but I also did Politics as a major
  273. 14:59Strand
  274. 15:00and you graduated in 1989.
  275. 15:04and I see you've been doing some
  276. 15:06teaching yes I worked as a volunteer
  277. 15:09teacher in West Africa I was there for
  278. 15:12almost three years in total from 1990 to
  279. 15:161992. how interesting what organization
  280. 15:20was that with it's not one of the major
  281. 15:22ones it's called Teach South all right
  282. 15:26yes I have heard of it it operates in
  283. 15:29several African countries doesn't it
  284. 15:31and what kind of school was it a rural
  285. 15:34Cooperative oh a rural Cooperative how
  286. 15:38interesting
  287. 15:39and what did you teach a variety of
  288. 15:41things in different years
  289. 15:44um I did with forms one to three mainly
  290. 15:48geography and some english with form
  291. 15:51five
  292. 15:52then in my final year I took on some
  293. 15:55Agricultural Science with the top year
  294. 15:58that's form six
  295. 16:01quite a variety
  296. 16:03also ran the school Farm how interesting
  297. 16:09before the interview continues you have
  298. 16:11some time to look at questions 28 to 30.

  301. 16:53and how did you find the whole
  302. 16:55experience
  303. 16:56I'll be honest with you at the end of
  304. 16:59the first year I really wanted to leave
  305. 17:01and come home oh why was that well I was
  306. 17:05very homesick at first and missed my
  307. 17:07family oh yes I can understand that and
  308. 17:10I also found it frustrating to have so
  309. 17:12few teaching resources but I did decide
  310. 17:15to stay and in the end I extended my
  311. 17:18tour to a third year right things must
  312. 17:21have looked up then yes we set up a very
  313. 17:24successful project breeding cattle to
  314. 17:26sell locally really
  315. 17:28and then after a lot of hard work we
  316. 17:31finally got funds for new farm buildings
  317. 17:33and you wanted to see things through and
  318. 17:36that's why you want to train to teach
  319. 17:37geography yes
  320. 17:39I've had a couple of jobs since then but
  321. 17:42I now realize I like teaching best
  322. 17:45and I chose geography because
  323. 17:48because it's my favorite subject
  324. 17:51and also because I think it has so many
  325. 17:53useful applications Well you certainly
  326. 17:56have had some interesting work
  327. 17:58experience
  328. 17:59I'll ask you now to go on to the next
  329. 18:01stage of the interview

  336. 18:55back pain given by Dr Paula clayberg of
  337. 18:58Liverpool's Wilton Clinic

  342. 20:03today's Health council is presented by
  343. 20:06Paula claiberg who is the chief
  344. 20:09counselor at Liverpool's famous pain
  345. 20:11clinic the Wilton Clinic Paula
  346. 20:15do you know what Prince Charles seby
  347. 20:18Ballesteros and Elizabeth Taylor have in
  348. 20:21common
  349. 20:23they all suffer from chronic back pain
  350. 20:26in fact bad backs are one of the most
  351. 20:29common health problems today
  352. 20:31affecting people in all walks of life
  353. 20:35the most recent available figures show
  354. 20:38that about a quarter of a million people
  355. 20:40are incapacitated with back pain every
  356. 20:43day
  357. 20:44and many sufferers don't know the cause
  358. 20:47or the solution to their problem
  359. 20:51the majority of our patients at the
  360. 20:53clinic tend to be women
  361. 20:56they are especially vulnerable because
  362. 20:59of pregnancy but also because of
  363. 21:02osteoporosis which I personally believe
  364. 21:05to be the major cause of problems for
  365. 21:07women
  366. 21:09I have many women patients who say they
  367. 21:12have completely given up exercise
  368. 21:13because the pain makes them so miserable
  369. 21:17but of course that starts up a vicious
  370. 21:19circle
  371. 21:21bed rest giving up exercise and
  372. 21:24painkillers are traditional responses to
  373. 21:27back pain
  374. 21:28but although there are many excellent
  375. 21:30drugs on the market at our Clinic we're
  376. 21:33beginning to realize the unique benefits
  377. 21:35of relaxation therapy
  378. 21:38other specialists in the field make a
  379. 21:41strong case for certain types of
  380. 21:43exercise but in our experience they are
  381. 21:46easily mishandled and can lead to more
  382. 21:49harm than good
  383. 21:51now let's look at some of the reasons
  384. 21:54why back pain is developing into such a
  385. 21:56unique Menace
  386. 21:58in general the body is pretty good at
  387. 22:00self-repair
  388. 22:02a strain or a blow to a limb though
  389. 22:04painful at the time generally resolves
  390. 22:07itself
  391. 22:09but the body's response to back injury
  392. 22:11can be very counterproductive
  393. 22:14when pain strikes we attempt to keep the
  394. 22:17back as immobile as possible which makes
  395. 22:19the muscles tense up
  396. 22:21research shows that they often go into
  397. 22:23spasm which causes further twisting of
  398. 22:26the spine
  399. 22:27a vicious circle is underway
  400. 22:31the second mistake we often make when
  401. 22:33stricken with extreme back pain is to go
  402. 22:36to bed and stay there
  403. 22:38although at the clinic we recognize that
  404. 22:40a short rest in bed can be helpful up to
  405. 22:43two days
  406. 22:45any longer makes our back muscles become
  407. 22:48weaker and unable to hold up our spine
  408. 22:51the pain therefore becomes worse
  409. 22:55another problem is being overweight
  410. 22:58anyone as stone or more overweight who
  411. 23:01already has back pain is not doing
  412. 23:04himself any favors
  413. 23:06though it won't actually set it off in
  414. 23:08the first place the weight will increase
  415. 23:10the strain and make things worse
  416. 23:14the British diet could be partially to
  417. 23:16blame for the increase in back pain
  418. 23:18over the last 10 years the average
  419. 23:21weight of men has risen by 11 pounds and
  420. 23:24of women by nine pounds
  421. 23:28so much for the causes and aggravations
  422. 23:30of pain
  423. 23:32but what can we do to help
  424. 23:37there are many ways in which simple
  425. 23:39day-to-day care can make all the
  426. 23:41difference
  427. 23:42the first point to watch of course is
  428. 23:44weight
  429. 23:45if you are overweight a diet will make
  430. 23:48all the difference
  431. 23:50also Studies have shown that just one
  432. 23:53hour sitting in a slouched position
  433. 23:55constrained ligaments in the back which
  434. 23:57can take months to heal
  435. 24:00at the clinic we have come to the
  436. 24:02conclusion that the major cause of the
  437. 24:05problem is not with the design of chairs
  438. 24:07as some have suggested but in a way we
  439. 24:10sit in them
  440. 24:12it can be useful to get special
  441. 24:14Orthopedic chairs but remember the most
  442. 24:17important Improvement should be in our
  443. 24:19posture
  444. 24:21another enemy of your back is of course
  445. 24:24your beds
  446. 24:25if your bed doesn't give enough support
  447. 24:27back muscles and ligaments work all
  448. 24:30night trying to correct spinal alignment
  449. 24:32so you wake up with a tired aching back
  450. 24:36try out an orthopedic mattress or a
  451. 24:39spring slatted bed
  452. 24:41research shows that both can be
  453. 24:43beneficial for certain types of back
  454. 24:45pain
  455. 24:47another Hazard for your back are the
  456. 24:49shock waves which travel up your spine
  457. 24:51when you walk known as heel strike
  458. 24:55a real find for our patients has been
  459. 24:58the shock absorbing shoe insert a cheap
  460. 25:01but very effective solution
  461. 25:04and you might be better off avoiding
  462. 25:06shoes with heels higher than one and a
  463. 25:08half inches
  464. 25:09though absolutely flat shoes can be a
  465. 25:12solution for some others find their
  466. 25:15posture suffers
  467. 25:17finally a word about the
  468. 25:19state-of-the-art relief the TENS machine
  469. 25:23a small battery-powered Gadget which
  470. 25:26delivers subliminal electrical pulses to
  471. 25:28the skin
  472. 25:30our experience indicates that your money
  473. 25:32is better spent on the more
  474. 25:34old-fashioned remedies
  475. 25:36that is the end of section four
