  1. 0:00hello
  2. 0:01and welcome to dr paul on strangulation
  3. 0:05in order to understand what happens in
  4. 0:08strangulation we first have to look at
  5. 0:12the way blood moves about the head
  6. 0:15there we have this
  7. 0:18carotid arteries marked ca taking blood
  8. 0:21into the head
  9. 0:22the vertebral arteries mark va taking
  10. 0:25blood into the head and the jugular vein
  11. 0:28in blue
  12. 0:30marked jv taking blood out of the head
  13. 0:34now when we compress it
  14. 0:37the jugular vein
  15. 0:39is closed
  16. 0:41very readily
  17. 0:43at 5.5 psi that's all it takes to close
  18. 0:46it
  19. 0:47the carotid artery take 11.1 psi quite a
  20. 0:51bit higher and the vertebral artery
  21. 0:53takes 66 psi to close
  22. 0:57so you need pavarotti standing on your
  23. 0:59neck to close them
  24. 1:01so
  25. 1:03blood can continue into the head but
  26. 1:05can't get out of the head so the head
  27. 1:08becomes over pressured
  28. 1:10death occurs due to cardiovascular
  29. 1:13congestion
  30. 1:15very important this a lot of people
  31. 1:17think deaths due to suffocation
  32. 1:20but cutting off the air but the trachea
  33. 1:23takes 33 psi to close it so again you
  34. 1:27need pavarotti
  35. 1:30now
  36. 1:33high pressure in the head causes
  37. 1:35capillaries to rupture
  38. 1:37particularly in the soft tissue around
  39. 1:40the eyes and in the eyes themselves
  40. 1:43this
  41. 1:44is called peticile hemorrhaging
  42. 1:47there you can see the little hemorrhages
  43. 1:50around the eyes tiny pin pricked size
  44. 1:52hemorrhages in red
  45. 1:58now
  46. 1:59another sign
  47. 2:01of strangulation
  48. 2:04is fracture of the hyoid
  49. 2:07and the cricoid cartilage now there they
  50. 2:10are in the neck hyoid bone cricoid
  51. 2:13cartilage
  52. 2:15these are difficult to fracture except
  53. 2:17by strangulation
  54. 2:20so
  55. 2:21you've got two signs one particular
  56. 2:24hemorrhages the pathologist always looks
  57. 2:27in the eyes
  58. 2:29and then they dissect the neck to find
  59. 2:32if the hyoid and cricoid bones fractured
  60. 2:35that will be
  61. 2:37confirmation that the person was
  62. 2:40strangled
  63. 2:41okay
  64. 2:42goodbye
