- 0:21iscribe with me here today just going to
- 0:23be taking some notes
- 0:24okay that's kind of a nasty cut you got
- 0:27there tell me what happened well um just
- 0:29before before I got here I was riding my
- 0:31bike and I I don't really know what
- 0:33happened but something happened to the
- 0:34gears and I fell off and landed on my
- 0:37outet like arms and somehow this
- 0:41happened okay and what uh surface did
- 0:44you land on was the Street sidewalk
- 0:46grass it was like on the curve of the
- 0:47sidewalk onto the street it was pretty
- 0:49hard did you heard anything else besides
- 0:51it I don't think so maybe a few scrapes
- 0:53and stuff but okay you can move your
- 0:56wrists okay and um do you have any
- 0:59medical problems I should be aware of
- 1:00are you pretty healthy normally um none
- 1:03that I can think of not on any
- 1:05medications not on any blood thinners oh
- 1:07I am on I do take a baby aspirin okay
- 1:10that's important to know and um do you
- 1:13have a headache a little bit all right
- 1:17where's that at front it's yeah it's
- 1:19mostly frontal okay how's your neck feel
- 1:22um I think it's a little stiff but I
- 1:24don't know if it's just cuz I'm holding
- 1:25it weird trying not to I don't know move
- 1:27too much okay last t shot I have no idea
- 1:32that's probably too long we'll take care
- 1:34of that today okay all right you're not
- 1:36having trouble breathing no nausea or
- 1:39vomiting I'm a little nauseous but I
- 1:41think it's just cuz I'm worked up guess
- 1:44okay and your knees and everything
- 1:46they're okay yeah that's all fine how
- 1:48about the bike um it's pretty much done
- 1:51pretty much toast total it okay um
- 1:57anything else about this accident you
- 1:59think I should know that I
- 2:00I don't think so okay you haven't been
- 2:02sick with anything recently like recent
- 2:04cough or cold or fever shortness of
- 2:06breath or Ving diarrhea okay sounds good
- 2:11let's take a look at it here well that's
- 2:14a kind of a nasty
- 2:16cut so um left cheek has a longitudinal
- 2:21laceration slightly
- 2:24irregular
- 2:26approximately uh 6 cm in length
- 2:30with minimal active bleeding are you
- 2:32tender to touch up here yeah a little
- 2:34bit okay anywhere else that's not soad
- 2:38not so bad some mild frontal tenderness
- 2:40without any obvious contusion go ahead
- 2:42and push against me does that hurt your
- 2:44neck no but if you push this way a
- 2:48little bit that way not so bad okay so
- 2:52some pain with motion against resistance
- 2:55in the neck tell me where it's the most
- 2:57tender in midline
- 3:00or off to the sides um off to the side
- 3:02like on the right okay so right
- 3:05paracervical
- 3:06tenderness and not saring the
- 3:09chest
- 3:14forward deep
- 3:18breath not tendering the back at all no
- 3:21over the kidneys nope no okay
- 3:27back 2 over 6 did you not get a little
- 3:30murmur
- 3:32M that's probably nothing but certainly
- 3:35not related to anything
- 3:36today pelvis okay yeah okay move your
- 3:40shoulders your elbows so no pain with
- 3:43any of that M okay no pain with range of
- 3:46motion either upper extremity but if I
- 3:49rotate the hips with the knees no
- 3:51problems no no pain with range of motion
- 3:53lower extremity
- 3:55either okay well um this is going to
- 3:58take some suturing and uh since you're
- 4:00on an aspirin and you whacked your head
- 4:03and sounds like you hit concrete or
- 4:06cement or something pretty hard we're
- 4:08going to be getting a CAT scan of the
- 4:10head no
- 4:11contrast and um with the discomfort in
- 4:14the neck we're just going to take a look
- 4:15at that two so also CTC spine no
- 4:19contrast and uh then I will be back and
- 4:22we'll suture that and I'll let you know
- 4:24what your CAT scan shows okay do you
- 4:26need anything for pain right now or are
- 4:27you okay um something pain would be good
- 4:31okay well um since we don't have a line
- 4:34we'll give you a little pain shot so
- 4:37four of more sub
- 4:39cute and then I'll get back with you and
- 4:42we'll Sut you
- 4:46okay well your CAT scans came back
- 4:49they're all fine there's no bleeding in
- 4:51the head the neck looks fine so um any
- 4:55soreness you have there is more muscular
- 4:57and probably cons just use some Tylenol
- 4:59ibuprofen at home or something like that
- 5:02um we uh sutured you up there and um for
- 5:07the scribes benefit uh it was a um
- 5:12uh what do we say 6 cm uh
- 5:16lastation uh it
- 5:18was U anesthetized with 1% Lidocaine
- 5:22with
- 5:24epinephrine uh 3
- 5:26CC's and uh sutured with
- 5:3060
- 5:31nylon number 12 interrupted
- 5:35sutures without any
- 5:38complication and the Basset TRC and
- 5:40dressing will be applied okay so those
- 5:43sutures will need to stay in for about
- 5:45five to seven days your doctor can take
- 5:47them out and um thinking that you should
- 5:50be okay with some ibuprofen at home that
- 5:52you can just use over the C and we'll
- 5:55give you some instructions on cleaning
- 5:56that and taking care of it and you
- 5:58should be good to go here here shortly
