  5. 0:12i'm here using my own solid chair and my
  6. 0:16assistant desiree is going to throw at
  7. 0:18me all these questions people have
  8. 0:21when they do
  9. 0:23amstela or use themselves here at the
  10. 0:25office for their pelvic floor
  11. 0:27dysfunction
  12. 0:29so first this is amselah
  13. 0:32this is the actual monitor that we sent
  14. 0:34the patient and we have it all set up so
  15. 0:36this year should i go ahead and start it
  16. 0:38yeah so i'm just going to press place
  17. 0:40start so this is a
  18. 0:42inaction thing
  19. 0:44and so it's already on
  20. 0:47position the patient press enter all
  21. 0:50that stuff so
  22. 0:53here we go and i'm gonna crank this up
  23. 0:56so i'm sitting here
  24. 0:59with the
  25. 1:01whole bottom of me
  26. 1:04not just the anus or the vaginal opening
  27. 1:07but that whole surface
  28. 1:09so
  29. 1:10vaginal opening in this and that tissue
  30. 1:12in between is being possible is on the
  31. 1:16chair i'm going to crank this up and
  32. 1:18what i'm looking for is a sensation
  33. 1:24in all of that area at the same time
  34. 1:30so here i am feeling it now about
  35. 1:43okay so now i'm going to position myself
  36. 1:46what we want to do is to make sure that
  37. 1:48the whole area is being
  38. 1:53address
  39. 1:54every time there is a contraction or
  40. 1:57there's that stimulation the whole
  41. 1:59inside
  42. 2:01feels like
  43. 2:03is contracted and lifted that's what it
  44. 2:05feels like to me is that how people
  45. 2:08describe it to you to those right do you
  46. 2:10think so that's the most common question
  47. 2:12is
  48. 2:13what does it feel like what is it
  49. 2:14supposed to feel like uh-huh so there is
  50. 2:17no supposed to
  51. 2:19because we both know like one of the
  52. 2:21things that happens
  53. 2:24so this is i'm cranking this thing up
  54. 2:30okay so i'm gonna i think i'm gonna keep
  55. 2:33it there not because it causes any pain
  56. 2:35but i feel like i'm i'm getting
  57. 2:38sufficient contraction does it feel like
  58. 2:41muscle skin no it doesn't actually
  59. 2:44because
  60. 2:46i can't describe what it feels like
  61. 2:48because muscle stim is more of a
  62. 2:50stimulation like this and this is more
  63. 2:52of a
  64. 2:53doing this and just lift it
  65. 2:59doing that and just lifting inside of me
  66. 3:02that's what it feels like i'm going to
  67. 3:03crank this up even more to see where we
  68. 3:05go
  69. 3:08so right now i feel like as i cranked it
  70. 3:11up i feel like i'm tilted
  71. 3:14my pelvis is tilted so i'm feeling more
  72. 3:17in the front
  73. 3:18than the back and actually on the inner
  74. 3:21thigh a little bit so that's when we
  75. 3:23have to adjust patients and that's what
  76. 3:25we do so i'm just going to sit back a
  77. 3:27little bit more
  78. 3:31i feel like i need to move a little bit
  79. 3:33to the right
  80. 3:36maybe that was a little too much
  81. 3:39how did you know
  82. 3:41how did i know because it wasn't this is
  83. 3:43definitely evenly distributed because as
  84. 3:45i move to the right i felt like it was i
  85. 3:48was feeling it on one side more than the
  86. 3:50other so i positioned myself a little
  87. 3:52bit more and now i'm there
  88. 3:55i with every contraction i feel like
  89. 3:58that whole surface is being contracted
  90. 4:02and lifted so i'm right now and
  91. 4:06i'm just gonna crank that up
  92. 4:09still
  93. 4:12and this is very good
  94. 4:14so one of the things we look at is are
  95. 4:17the patient's feet on the ground
  96. 4:20because we want to make sure that the
  97. 4:21feet are on the ground and sometimes
  98. 4:24people actually so this
  99. 4:26you can hear the sound it's making
  100. 4:29because that's stimulating it
  101. 4:30differently it's relaxing it's
  102. 4:33stimulating and it's prepping for the
  103. 4:35few i think we counted it a few seconds
  104. 4:39and then it starts going through that
  105. 4:41phase again
  106. 4:43there are different parameters that we
  107. 4:45can set
  108. 4:46but that's how it feels but back to the
  109. 4:48feet the feet are on the ground
  110. 4:51and sometimes people feel it on in one
  111. 4:54leg more than the other right
  112. 4:56we've had that issue going on or they
  113. 4:58feel like going down their leg going
  114. 5:00down their leg all the way
  115. 5:02posteriorly so that all depends on what
  116. 5:05muscle is being activated
  117. 5:08or it shouldn't be any of those muscles
  118. 5:10so we need to position that just shows
  119. 5:12the imbalance where we've done a video
  120. 5:15on how your abductors your hip rotators
  121. 5:19and your hips stabilizers play a role so
  122. 5:22if they're not even so now it's going up
  123. 5:25again so when they're not even you're
  124. 5:27going to not feel
  125. 5:29the contraction evenly so people realize
  126. 5:32that now does this happen every time the
  127. 5:34patient comes in or is it typically at
  128. 5:36the beginning
  129. 5:37it's
  130. 5:39it can happen every time but it's
  131. 5:42usually towards the end of the session
  132. 5:44because you're sitting there for 30
  133. 5:45minutes that people either collapse or
  134. 5:49they do the opposite they're trying to
  135. 5:51hold themselves up and it throws
  136. 5:52everything off okay so
  137. 5:55so do you see how when because you need
  138. 5:57to sit here for 30 minutes now we have
  139. 5:59questions that we put and we have a
  140. 6:02timer that we have we come in and we
  141. 6:04check the
  142. 6:05patient's positioning because we want to
  143. 6:07optimize the session
  144. 6:09to the best and best advantage for our
  145. 6:13patient but as they fail and they kind
  146. 6:16of they get tired and they sit right
  147. 6:18here like right now as i'm sitting here
  148. 6:20i'm hardly
  149. 6:21i'm lifted up
  150. 6:23i'm not sitting on my pelvic floor
  151. 6:26evenly as i go up
  152. 6:30i definitely feel it on that and now i'm
  153. 6:32gonna arch my back
  154. 6:36and once again it's not even
  155. 6:38distribution which makes sense so you
  156. 6:40need to be seated on your seatbelt and
  157. 6:43basically
  158. 6:44checked periodically to do that any
  159. 6:47other questions that people have
  160. 6:50um we've done other videos but i don't
  161. 6:52think you said it specifically in this
  162. 6:54one what does it do
  163. 6:56so what this machine does it actually
  164. 6:58produces 400 kegels a minute
  165. 7:03so imagine 400 times 28 minutes 30
  166. 7:06minutes 28 minutes is how many people
  167. 7:10you get with every session why is that
  168. 7:12important is that only for incontinence
  169. 7:15well the machine is fda approved for
  170. 7:16incontinence why producing kegel
  171. 7:19so i
  172. 7:22use the machine for every time i
  173. 7:25see
  174. 7:26pelvic floor dysfunction
  175. 7:28that exhibits
  176. 7:31signs of weakness in the pelvic floor
  177. 7:34so that becomes in conjunction with
  178. 7:37whatever else we're doing not everyone
  179. 7:40who receives this has
  180. 7:42this so-called
  181. 7:44incontinence
  182. 7:45necessarily pelvic floor dysfunction
  183. 7:48comes in so many shapes and forms we've
  184. 7:50done a whole video on that
  185. 7:52written blossom so many cases of pelvic
  186. 7:55floor dysfunction incontinence is one of
  187. 7:57them and not everyone's story so we can
  188. 8:00have we sell the floor without having
  189. 8:03incontinence that's the whole story
  190. 8:06it does contraction for you
  191. 8:09what other questions are typically asked
  192. 8:12um
  193. 8:13do you keep your clothes on which you
  194. 8:15obviously have on yes you don't need to
  195. 8:17take it and no nothing comes out of this
  196. 8:20table
  197. 8:21this chair
  198. 8:22i'm sitting on it it actually uses the
  199. 8:26electromagnetic kind of like mri that
  200. 8:28technology
  201. 8:30and that is being used actually right
  202. 8:32now we have a company that uses uh does
  203. 8:36neurotransmitters neurostimulators i'm
  204. 8:39sorry
  205. 8:40that are testing the machine to see if
  206. 8:43it impacts
  207. 8:44their device or not so
  208. 8:47you know we're glad to be a part of that
  209. 8:49research
  210. 8:50and
  211. 8:53hopefully benefiting other people
  212. 8:55any other questions that are typically
  213. 8:57asked
  214. 8:58how many times does it take
  215. 9:01um
  216. 9:03technically and typically in our office
  217. 9:05i can tell you about our offers we do
  218. 9:08about
  219. 9:10no more than 10 sessions but our
  220. 9:12sessions are always followed with the
  221. 9:14breathing
  222. 9:16reading
  223. 9:18instructions and practices which we have
  224. 9:20other devices that we connect to the
  225. 9:22patient to see the quality of their
  226. 9:25contraction of their uh
  227. 9:28diaphragm
  228. 9:30and we follow them with other movement
  229. 9:32exercises because we feel that that is
  230. 9:34necessary for people to understand what
  231. 9:36they need to do
  232. 9:40another thing that
  233. 9:41we actually do that is
  234. 9:44i think it's important for people to
  235. 9:46realize is that
  236. 9:49a lot of times i would say at high um
  237. 9:54almost everyone here
  238. 9:56with actual
  239. 9:58part of their
  240. 9:59dysfunction
  241. 10:01part of it was incontinence or having
  242. 10:03difficulty different types of
  243. 10:05incontinence they have cleared
  244. 10:08some of them first visit some of them
  245. 10:10after three or four
  246. 10:13visits they start noticing like 50 60
  247. 10:17improvement but the issue i have
  248. 10:20is this machine by itself can only do
  249. 10:23cable you have to accompany what you're
  250. 10:26creating with how to move and that's
  251. 10:29where
  252. 10:30i think people need to realize the
  253. 10:33machines that performance unlike what
  254. 10:35the
  255. 10:36factory and the
  256. 10:38manufacturer says they say well you know
  257. 10:41you need to maintain it every you know a
  258. 10:43couple times a year or maybe you know
  259. 10:46once every six to ten months in reality
  260. 10:49i think if you do the exercises to hold
  261. 10:53what you're created you shouldn't get
  262. 10:55that properly and that's what we do in
  263. 10:57our office we
  264. 10:59we
  265. 10:59implement other exercises relevant to
  266. 11:02the dysfunction to empower the patient
  267. 11:05with the tools necessary to proceed
  268. 11:08what other questions that was a long
  269. 11:10answer was
  270. 11:11but it had to be
  271. 11:13i think that covers it that covers it so
  272. 11:15it's a handful of questions that people
  273. 11:17have there
  274. 11:19very good so the session's about 28
  275. 11:21minutes i'm a minute 17.3
  276. 11:25so i've got 17 more minutes to do this
  277. 11:28so
  278. 11:29if there's no more questions we can stop
