- 0:26transfusion can cause many complications
- 0:29one of them is volume overload and this
- 0:32can cause heart failure
- 0:34because the heart cannot compensate to
- 0:36the increased volume
- 0:38that occur after transmission
- 0:41the sciom symptom
- 0:42of this volume overload is they will
- 0:45have increased blood pressure and also
- 0:47they have difficulty in breathing
- 0:49because the fluid has filled up their
- 0:51lung
- 0:52and also heart failure
- 0:54how do you want to prevent this from
- 0:56happening especially for elderly
- 0:58patients or patients with all the liver
- 1:00heart and renal disease we usually
- 1:03slowly transduce them with backward
- 1:05cells if they need it okay we don't
- 1:07transduce them with whole blood if they
- 1:10don't lose blood we don't transfuse them
- 1:12with whole blood we transfuse them with
- 1:15bad red cells and then we do the
- 1:17transmission very very slowly and
- 1:20monitor the patient very closely
- 1:22and in certain situation also we give
- 1:25them diuretic therapy
- 1:27diuretic therapy is a drug given to a
- 1:30patient
- 1:31to get rid of their fluid overload so
- 1:34this drug will make them pass urine very
- 1:36frequently to make sure they don't have
- 1:38volume overload okay that are the some
- 1:41complications that i have discussed if
- 1:44you want to know more about it or you
- 1:46want to know other complications you can
- 1:48read it in the book or we can discuss it
- 1:51during classes
- 1:53before i finish this lecture i just give
- 1:56you a few conclusions
- 1:58the first one is transfusion is an
- 2:00useful procedure and in many situations
- 2:02it can it can save lives however it is
- 2:06not without complications
- 2:08so good and wise clinical judgment is
- 2:10crucial before making a decision to
- 2:12transfuse a patient
- 2:15the health professionals
- 2:17involved in transmission service
- 2:19including nurses
- 2:21need to understand the risk of
- 2:23generation
- 2:24and their expertise in providing
- 2:26self-registration is very very vital and
- 2:29as nurses you also need to be aware what
- 2:32are the complications that can occur
- 2:35after transition or during transmission
- 2:38and you need to be aware
- 2:40of the signs and symptoms of
- 2:42transmission reaction because you are
- 2:45the one who are usually with the patient
- 2:48most of the time i just want to end this
- 2:50with a saying a blood translation is
- 2:53like a marriage it should not be entered
- 2:55upon lightly blindly or wantedly and
- 2:58definitely should not be more often than
- 3:00it is
